General fixes based on players feedback for Oasis Interlude+

Changed

1. Move Fiend Harthemon spawn to corrected Outlaw Forest coordinates so raid teleports and client raid data match.
2. Improve PlayerAugment with per-roll Adena pricing, stop-on-skill Continue/Stop UX, Focus options, and in-place rolls to avoid inventory-spam lag.

Fixed

1. Fix Blessed Olf's T-shirt enchant always failing at 0%
2. Fix KTB instance Lost Raid IDs triggering world teleport-to-raid by gating TeleportToRaid to default-world spawns only.
3. Fix Rare Pouch giving 100% S-grade recipes by restricting drops to 60% recipes only.
4. Fix Pa'agrio's Fist enchanted Power leaving current CP above max by applying maxCp first then HealCPPercent instead of COMBATPOINTHEAL.
5. Fix Resist Aqua/Wind/Fire Power enchant overriding MP cost instead of element resist.
